Meridian Hearing is committed to keep your personal and health information safe and confidential.  Your privacy is very important to us.

What Type of Information Do We Collect About You?

To provide you with quality health care, we collect both personal and health information from you.  Your name, date of birth, address, and extended health insurance numbers are examples of personal information.  Your health history, the records of your visits to the Centre and what health care we provide to you during those visits are examples of your health information.

Do We Share Your Information with Anyone?

Meridian Hearing does share your personal and health information with your family and/or any referring physician, who is part of your health care team for the purpose of your continuing care.  Your information is also shared with agencies that fund your hearing aid, for example, ADP or health insurance companies or the Workplace Safety and Insurance Board.

How is Your Information Used?

The information we collect form you is used to:

  • Provide you with quality hearing care and follow-up care in the community.
  • Carry out quality assurance to help make us better.
  • See how we are doing – through patient satisfaction surveys.
  • Comply with legal and regulatory requirements.
  • Contact you for your regular visits or any follow-up required.
  • Inform you of educational, health and wellness, and service opportunities and options.
